First Visit

sealants-lg.jpg

We are excited to get to know you! At Flossed Family Dentistry, your first visit is an opportunity to get to know you and your dental needs. Plus, this time gives you a chance to meet our team and find out more about our office.

After the evaluation, our treatment coordinator will review your custom treatment plan that is specific to your needs and to the overall health of your mouth. Then the patient coordinator will go over what is to be expected at each appointment, fees, and answer any other questions you may have.

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

Payment

We are committed to providing dental care that is both high quality and affordable. Payment options include cash, Visa, MasterCard, our in-Office Discount Plan and participating insurance.

We accept Care Credit with 6 & 12 months Zero Interest.

New Patient Forms

Save time, take care of the paperwork now. Our New Patient Forms are now available online so that you can complete them at your convenience, giving you time to carefully review our policies and procedures without having to do so at our office. Click on the "New Patient Forms" button to download and fill out the form, then bring it with you to your first appointment.

Download our fillable PDF to type your information in.  Print the forms and then sign them.  Once you've completed your forms scan them or take a picture of them and then send an email to info@flossedfamilydentistry.com with the subject line: [New Patient Forms]

Insurance

Our office is committed to helping you maximize your insurance benefits. Because insurance policies vary, we can only estimate your coverage in good faith but cannot guarantee coverage due to the complexities of insurance contracts. Your estimated patient portion must be paid as per the contract agreement.

As a service to our patients, we will bill insurance companies for services. If you have any questions, our courteous staff is always available to answer them. If your insurance covers orthodontic treatment, you will receive the benefit of reduced personal costs. Many insurance policies have a lifetime orthodontic benefit that is distinct from regular dental insurance. Insurance policies vary, so we will review your insurance to determine the appropriate course of action.

Once treatment has started, we will file your claims. To help us with your insurance filing, please complete the insurance questions on our New Patient Registration Forms. Flossed Family Dentistry accepts most insurance plans. Please have your insurance information accessible when scheduling your first appointment - that way we can verify your insurance benefits and minimize your out-of-pocket expense! If your plan requires a dentist referral, please obtain one before scheduling your complimentary consultation to avoid treatment delays. Please bring your insurance card and information to your consultation appointment.

Insurances we accept:
DHA (Dental Health Alliance), Aetna, Sun Life Financial, Mutual of Omaha, United Concordia, Humana, Unicare, Blue Cross Blue Shield, Careington, Cigna, Delta Dental, Guardian, Metlife, Principal, Assurant, UCCI TriCare, United Healthcare.
